Note: Patient refused the shots and didnt meet the immunizations requirements!, Civil Surgeon- Mid - I- Qwik Dr. MH Bacchus

Hello, I just want to ask if Dr. Arnold is a licensed doctor? Is he not a fake one? Just trying to ask about this. I really got mad with the Mid-I-Qwik Dr. because we are trying to explain to them that what I need is only a TD Shot, but they want me to get all the shots that they required, MMR, HIV, Hepa B, Skin test,etc. and it seems they are get mad at me also because I didnt believe in them and I didnt take the shots, they want me to pay $350 for laboratory exams. But it seems expensive to us so, we just take the TD shot for now and we will just come back after if I really need more.I really questioned it why? and they said that if I only get the TD shots the homeland security department will require me to do all the immunizations. But I got the TD Shot only and they gave me the supplemental form of record but they put a note there that the patient refused the shots, and didn't meet the immunizations requirements. I'm wondering why I didn't meet the required immunization thing and I didnt refused it either? Please everyone who already done there vaccinations I need your advices, help and any recommendations. Thanks alot!

Timeline

Most people require TD, MMR (2 doses) and Varicella (2 doses). Sometimes TB skin test is required but most people did not get it and no one requested them to do it. You can get MMR and Varicella done by any doctor (cheaper at county health clinic etc.) and take the proof to the civil surgeon for the vaccination supplement.

Since your supplement was noted as not complete, maybe you should get at least MMR and Varicella done and then send the proof to Dr. Arnold so he can give you another vaccination supplement page. I believe he is a real, licensed civil surgeon and others have gotten the supplement done by him and they successfully got the green card.

Maybe others with more recent experience would also like to comment.

hello ELW, I already done MMR and Varicella in the Philippines. But my vaccination record from the Philippines advice only to have a td shot. and no more other shot that I needed. I really got confused of this things.

Look for other doctor listed in the USCIS website. Best you can do just right now. Do you have the form of vaccines from your medical in your country?

Show to them. If you didn't bring one, maybe you can call to the doctor that made your medical in your country and he can send you a fax or something with your records.

Yep I had a vaccines record from our country. They saw it and they read it. and its really very clear the td shot box is checked. thats what im thinking for why they are not satisfied with the documents that I have from our country. maybe they want me to pay 350 bucks. :wacko:

  3. Send both forms to Dr Arnold's office and explain what happened. They will consolidate them onto one form.
  4. Congratulations on refusing unnecessary treatments. Civil Surgeons Offices are not immigration experts. They misinterpret USCIS regulations all the time. Good for you for speaking up.

Actually, unless there are 2 Dr. Arnolds at that address, I think he's a he. When I used that office to get the supplement completed, i wanted to make sure it was legit as well (i felt secure with all the posts here , but felt like it would be silly not to investigate on my own before sending cash to Texas), so I looked up the name on the Blue Cross site, and googled the name. There's a Dr. Lloyd E. Arnold at that address in Austin (and he's rated quite well as a care giver on several sites, btw).

Regarless of gender - this is definitely legit, and probably the best deal out there to get this form. All the local civil surgeons I called in my area wanted from $100 to $300 just to fill the darn thing out. And Dr. Arnold's office had our forms back to us in less than 2 weeks - well worth the wait for the savings.
