For the Medical Exam at SLEC:

What to bring:

1. Appointment Letter

2. Passport

3. Passport Photos (three (3) 2x2 passport photos as stated on the DOS website [CLICK HERE] (referenced on page 3 of Packet 4)

4. Payment of $95 USD (paid in pesos / $95 USD equivalent)

5. Ballpoint Pen

6. Tissue

7. Sweater or Jacket (SLEC is air conditioned and cold)

NOTE: Beneficiaries please bring or know your sponsor's personal infomation (address, phone number, and ssn #) for the SLEC forms. Don't be alarmed because the beneficiary's passport will be taken at SLEC and delivered to the USE along with your medical exam results prior to the visa interview. Beneficiaries please eat beforehand because SLEC prohibits food or drinks inside of the facility. Guest(s) and family member(s), except the beneficiaries child(ren), will NOT be permitted inside of SLEC. REMEMBER - The medical exam is a two day process. Day 1 is the medical exam and Day 2 is the vaccination. At the end of Day 2, SLEC will give the beneficiary their X-ray film and the vaccination worksheet/record (DS-3025).

1. Any Major operations?

2. Do you have Asthma?

3. Are you allergic to any medicine or food?

4. Do you smoke marijuana?

5. Do you smoke cigarettes?

6. Do you drink alcohol?

7. Have you ever had Chicken pox? (if yes) Where are the marks?

8. Have you ever had an abortion?

9. Have you ever attempted to commit suicide?

10. When was your last menstrual cycle?

11. What are those scars? How did you get them?

12. Do you have dentures? Any permanent fillings?

13. How many pregnancies? Any miscarriages?

14. When was the last time you had sexual contact? How was it?

p.s. - The "How was it?" follow-up question in #14 is a joke... Hahahaha!!! But they do ask when was your last sexual contact.

Regarding question #12 ---- is it ok to have dental braces? since i have those and i cant remove it unless i tell my orthodontics to remove it.

Jan. 31, 2009 - Actual Drive test passed "got my Driver's License"

Feb.25, 2009 - Took NCLEX-RN -- PASSED--- Anais,RN

you will just answer that questions, just answer yes and no lang, they wont mind kung may braces ka, wala lang yung question na yun. di nila papatanggal sayo.

p.s.

you have to know your last menstrual period, you will be asked this a million times during the medical. and if u can avoid, dont go there if u have your monthly period.. :thumbs:

06-24-08 - applied for SSN change of last name ( 07-02-08 - received SSN card with married name)

08-07-08 - AOS packet sent thru Fedex

08-08-08 - AOS packet received by USCIS

08-14-08 - NOA 1 date for AOS, EAD and AP

08-25-08 - Biometrics schedule received

09-05-08 - BIOMETRICS APPOINTMENT

09-08-08 - TOUCHED (I-485 and EAD)

09-17-08 - I-485 transferred to California Service Center

09-19-08 - I-485 TOUCHED (received mail same day about the transfer too)

10-03/06/07-08 - I-485 TOUCHED

10-21-08 - I-131 AP APPROVED

10-22-08 - I-765 EAD CARD PRODUCTION ORDERED

10-27-08 - RECEIVED AP in mail and I-765 touched again online

10-29-08 - Notice of Approval for I-765 was sent out today

10-30-08 - Notice of Approval for I-765 touched again then RECEIVED THE EAD CARD the same day

Denture are false teeth.

Dentures and braces is like comparing apples to oranges.

Suggestion:

If you have head gear be sure to wear it to the medical interview, It looks so cool. Plus it makes your cellfone signal better
