I may be wrong, but I think you need to send 2 passport size photograph of both USC and non-USC and have to write name at the back of the photograph.

Yes, the pictures are needed with the names on the back! :thumbs:

One more thing... I assume the birth certificate is for the USC and that is is your "proof of legal status in the USA"...

I also included a full copy of my passport. Overkill, but my birth certificate was from the 70's and looked like a fake ;).
