We just had our I-130 approved in Tokyo. We are preparing our documents for the final interview, but we need a medical exam. According to the U.S. Embassy-Tokyo website, the required medical forms can be obtained at the specified medical clinics (clinics approved by the U.S. Embassy). However, according to several of the medical clinic websites, we have to bring the medical forms to the examination. Is there anyone who did their medical checkups that knows the answer to this problem? Where did you get the forms? I couldn't find any medical forms on the U.S. Embassy website. Are there any downloadable forms?
