hello there

green card holder here. i have 18 months baby. mom mom ( tourist visa holder) wants to take baby for vacations to poland. baby is born here in US and has american passport. my mom had my maiden name (different then baby) and polish passport.

My husband is american citizen.

what papers do i need to provide for my mom to be able to take my baby girl for a month to poland? any info will help us! thank you in advance - Nika

I believe you will have to provide your mother with your child's passport as well as a signed and dated notarized letter from you and your husband allowing your mother to take your child out of the country. In the letter I would mention your mom (spell out her full name) has approval to take said child (full name again) on such and such date to Poland for such and such time.

At the bottom of the letter you provide your child's name and passport number, as well as your mom's name, passport number, flight numbers (if available), address and phone number. Made sure both you and your husband sign the letter and date it in front of a notary. Notaries are available at any bank free of charge.

That's what I have to do when I send my son to visit his dad, to and from the US. Without that letter your mom will not be allowed to leave the country with your child, or return. So make sure you have both leaving and arrival dates indicated on the letter.

I think you may have to get an Apostille. A notarized document may not be good enough in Poland. It only costs a couple bucks... and a trip to a Secretary of State office, but better to have this than face problems with returning.

In Poland, our notarized documents don't mean anything.

Also, just to be on the safe side, I'd call the local PL embassy here in the US, if there's anything you need to do. I know the baby is a USC, but just hope Poland doesnt have any paper requirements for like you to fill out that its ok for grandma to return with the baby.
