I have another question! For AFFIDAVIT OF SUPPORT. FORM I-864. do we need to get it notarized??? Also what do we need to sent with it FORM W2.. paper from work that I work t here and what else??? does anyone know? I need an answer ASAP! Please

September 18,2007 ---> Sent I-129F to California Service Center

September 20 ---> USCIS sent out a recepit(NOA1)

I never received the envelop with NOA1..( I think it got lost)

September 24 ---> Check Cashed

September 25 ---> TOUCHED

December 4 ---> I called USCIS and they gave me the receipt number...

FEBRUARY 7, 2008 WE WERE APPROVED!!!!! 142 days from filling date.

No need to notarized. Employment statement, Bank statement, ITR.

Your last three years tax returns with W2s or Transcripts, and a letter from your employer. If you are using assets then you will need bank statements and proof of other value of assets.

The check or money order is made payable to:

Department of Homeland Security.

Hi everyone! These papers are driving me crazy already... I have another question please... If we have the medical paper from overseeas, do we still need to go to a doctor so he can fill out form I-693A???? I don't understand anymore!

Yes, a US Civil Surgeon must transcribe your vaccination records onto an I-693A (if the medical was done within the past year for the K1, the K1 medical does not require vaccinations). If required vaccinations are missing, they must be received.

According to the following pinned topic, you do not have to have the medical paper transcribed to form I-693A, if you still have the Form DS-3025 (Vaccination Supplement) from your K-1 medical, if it was done less than one year ago.

I'm hoping this is right, because I just sent a copy of DS-3025 with my AOS. I guess we'll just have to wait and see! There seems to be some contradictory info on visajourney about this...I suppose if you want to be extra 100% safe you could have a doctor do the I-693a for you.
