Hello great Vj's memebers,I greet you all,really want to thank everyone of

you who joined us to celebrate our victory of my husband coming home.

My God will do the same for all those who are still waiting patiently.

My question is How do I go about my medicals for AOS?He came with the vaccination sheet

from kamoross clinic Lagos,he took 2 dose of the vaccine.Can we send that vaccination sheet with the package?Or do we consult this INS civil doctors?Please for those who have gone through this, what is required for the AOS medicals,pls let me know.

Hi , congratulation

would have gotten form DS-3025 from the doctor at the timeof your medical. However the information on this form needs to be transfer to the form I-693A by a civil surgeon. Google Civil Surgeon in your area call them up and see if one will do it for a small fee shouldn't cost more than $50

mine cost 40$ and u should tell doctor that u already took the shots , because every doctor u will go to will try to give you at least 2 shots which cost 195$

so after that i got the form I-693 sealed

i told doctor that i took some when i was child and other i took them the day of my medical process

i guess i will be may filler

mine was really different i got no DS-3025
