contents of packet 3 (K1) sent to consulate ?

Hi All

The VSC sent us NOA2 in 59 days but we have already lost this benefit due to the deeds of the Consulate at Mumbai.

They told me that they send the packet 3 5 weeks back and through regular post. No regular post reaches anybody here.

Now, they ask me to download the forms from the website and send it to them. Since I dont have a formal checklist, I

seek your generous help in tiding through this scenario.

1) What are the contents to be sent in packet 3 ?

(a) Application Statement

(B.) DS 230 Part 1

(c.) Checklist ? after ticking all the boxes to signify I have duly collected those items and they are with me ?

2) I dont have birth certificate. I have been using my school record certificate till now which has my birth date. But, they need names of the parents and

place of birth. So, do I need to get an affidavit done before a first class magistrate ?

3) Since I am a divorcee, I guess I only need to send my divorce certificate ..

4) Police report - what would be the format ? is it just to prove my domicile ? They also need one certificate from passport office.

And finally 5) I - 134 .

Do I need to send the items 1 (a to c) and 5 only to the consulate right now ?

Pls help !
