To all

As part of my K-1 medical exam I had to complete my "shots". The doctor in Vancouver used the form provided on my Packet 3...which looks alike like a I-693A, but is not an I-693A (all the data and lines are the same except the form is oriented landscaped vs. portrait). Does anyone see an issue with submitting this (For AOS) on the back of a I-693A with a note attached to the front?

The instructions state that K1s have to submit a vaccination supplement filled out by a designated civil surgeon. So it might be an issue if you don't submit it, at the discretion of the USCIS. See link for more info.

You most likely got a vaccination worksheet from Vancouver, not a supplement. Some have been successful with submitting a copy of the worksheet with their AOS, some have not and have had to supply a supplement because of receiving a RFE, some didn't risk it and submitted a supplement. I believe the I-485 instructions do ask for the supplement, which must be completed by a US civil surgeon, but again, some have been successful with just the worksheet.

Yes, the vaccination worksheet is landscape - 11 by 8 (rather than 8 by 11 as for most normal documents.) You will see the form number DS3025 at one of the corners (bottom right I think.) We only submitted a copy of this and took a chance on not submitting the I693 - and we were not asked anything - so you can take a chance (know that if they want it - it will cost a delay) so how lucky do you feel? Good Luck

Ya, it is the luck of the draw, I'm trying to talk a local CS into just transfering the info on to a 693A, will see

Either case it could still be who gets the packet at USCIS, as usual there is no standard

I submitted my AOS package less the I-693 supplement because my wife did not get a copy of anything whilst doing her medical in-country and when she asked for copies was told she didn't need any. All she had was the infamous "sealed brown envelope".

I had to conclude that she had received all the required vaccinations since one cannot be expected to "supplement" what one does not have (a record of vaccinations).

I included a letter in my AOS simply saying so. Some say it was luck, others say it was gambling, but I simply believe it to be the logical choice given the circumstances.

We never were asked via an RFE for the I-693 and we subsequently received an approval and green-card without interview.

I submitted a photocopy of what I got at my K-1 medical, and was asked for the 693A at the interview... If i had had a less lenient interviewing officer, my AOS approval would have been delayed... Its mostly just a question on if you want to gamble that delay...

you have the vaccination worksheet and the I-485 asks for the vaccination supplement.
