Yeah you are right! when i called it is only $15.00. But i dont think they will transfer my vaccination record to the suplement form.

No unless they have a US Civil surgeon on staff. You will then take the record that of vaccination to a US Civil surgeon for transfer to a supplement.

From my experience it is a bigger hassle, but cheapest to go to the Civil Surgeon first, go through your history, and then he or she will tell you what vaccinations you need. Then you contact your board of health, tell them you want to talk to someone about adult vaccinations. You will find the prices much cheaper and then after you get all your shots, go back to the Civil Surgeon's office (or fax them the copies of your new vaccinations) and they should then prepare your vaccination supplement for you. All this you have to discuss with the Civil Surgeon though. And of course this depends on the person's character...some civil surgeons are jerks, some are very nice.

Hi, i got my TD shot today from the public city health department and it was no fee "FREE". thanks for ur suggestion.

I'll receive another RFE (not sure when I'll get it in my mailbox), they just sent out on 12th as said in the CRIS email!! Not sure what it is about yet, but i bet 90% is about the medical...well, I think I'd better to check with the Health Dept what can I do soon...

yes, going to health department for shot are cheaper actually its free i go there

to do for my son to update his shots but me i go to my husband doctor and he

charge me $180 they dont accept insurance its sucks!

Few days ago, I've got my TD shot from the public city health department and it was free...and we sent the documents to

Dr. L E Arnold from texas to transcribed all my vaccination record to the suplemental form the I-693 . . . And today, i recieved the package from Dr. L E arnold the sealed envelop inside "suplemental documents and copy for my files" Wow! we only pay $20 to transcribed my vaccination record to suplemental forms by the Civil Surgeon. the TD shot was no fee . . . So am so happy . . .

skin test and blood test are defferent. thats for TB test. TB is tuberculoses.
