G-325A form Q;Other names used

hi everyone ...I have a question for G-325A form...on the form there is a question says "OTHER NAMES USED".I understand that I have to write my maiden name,but I also have changed my name before.My parents got divorced and they changed my birthname to my name that i use now.it is not on my personal regal record but it is on my family regal record....should i write it on te G-325A form?

Yes - All other names you used should be mentioned - they will do a name check on all of these. This is routine. Good Luck

Like you guys suggest I will write all the names I used before ,on the G-325A form..but Do I need any types of document shows my name has changed??My old name isnt on my Family Registry (Family Record) any more so I dont know how to proove my name has changed...

No, you do not require a document to show your name change, unless it was by a previous marriage. Don't over-analyze this - you are going to stress out. Good Luck

hi motu ,thanks for your advice...

so you mean I dont have to have documents that shows all the names??Because i will need to put 4 diferent names on the Form and I can proove only 2 names by my family registry record...dont I need anything to explain why my name has changed?????? I know Im stressing myself over this but Im afraid they might send me back the paper works later....

My wife had different names but the only thing she had included in her documents was her Passport - they are more worried about other things at this point and when they do a name check all permutations and combinations of all the names are used (this is what I have heard) - they don't need any official document to show the existence of these names. Good Luck
