If you are petitioning for a fiance, the answer for Husband or Wife is "none". The rest of the information about your spouse is N/A because you don't have one.

There asking your family name which in most cases is your fathers surname and there asking for your mothers maiden name....Not sure if thats correct or not though.

There asking your family name which in most cases is your fathers surname and there asking for your mothers maiden name....Not sure if thats correct or not though.

That's in a different place. They ask for family name for each person to which the document refers, so there could be as many as four separate family names. Put the one appropriate to the person referred to.
