1. The name you are known as and use. If that is your married name you would put your married name.

3. Write your full name in your native language.

9. As you are applying for a CR-1 spousal visa you would mark 'married'

14. That is asking for name of spouse. Whatever name your spouse is legally known as.

17. Just put the name that your mother was born with. It doesnt matter if it is the same, you still need to put it.

As it states on the form you put N/A for any question that is not applicable.

You should of received the DS-230 by now. Give them a call using the autmated system on (603) 334-0700 and then press 5 to speak to an operator to let them know you have not received it yet.

You can self generate the barcoded cover sheet if you are confident in doing so.....download and complete a DS-230 from USCIS website and send this along with the self generated barcode sheet to NVC. You are welcome :) You did replace the relevant info on the coversheet, yes??? You also need to make sure that you send everything that is required along with the DS-230. Check here for the instruction packet that is applicable to Pakistan http://travel.state.gov/visa/immigrants/types/types_1309.html I would also double check with someone else going through Pakistan to make sure you have all that NVC will require.

In case you hadnt noticed the address is now outdated on the coversheet as they were done some time ago by another member. There is a reference to it in the actual post so just making sure you noted that.

The address to send the completed DS-230 with barcoded cover sheet is

National Visa Center

ATTN: CMR

32 Rochester Avenue

Portsmouth, NH 03801-2909

Your case number must also be clearly written in the top right hand corner of ALL documents. Place the barcode sheet at the front of your DS-230 package.

And yes, if you know for certain that NVC has generated your DS-230 already (from your original post you say it was mailed from them) then yes you can send this package to them.

As long as you replace with your info and put the address I posted in place of the one in the sample barcode sheet you did just fine. It will not delay your process. Many on here have used James Shortcuts successfully. I havent noted any rejections either from using them.

If you click on Pakistan to the left under your ID it will bring up members in Pakistan. The ones having done or are doing CR-1 are the ones that can tell exactly what you are required to send in with your DS-230.

As I am in Australia I was only required to submit the DS-230 part I and a copy of my passport. Other country's are required to send a lot more including DS-230 part II. If you are unsure it is always best to ask someone that is from your country. Members here are only too willing to help. All you have to do is ask.

Could someone really pin this down? I have seen several different answers and it is confusing. On the DS230, Number 14-Name of Spouse (Maiden or family name). If the immigrant is male would that question's answer be his wife's maiden name or her current married name if that is what she uses on her passport, driver's license, etc.?

Thanks for any clarification.

8-12-2004 I moved to New Zealand(married my Kiwi in US 5/04)

Firstly.........KamalJohal

The question is asking about your spouse. If you are married you should put your spouses name. It is not differentiating on whether it is the husband or wife. Answering N/A would be incorrect.

Sadie

My interpretation is one can put either the maiden or family name which ever one is used by the wife. Some wives elect to keep their maiden name
