Last Name Question

OK... I have looked through the guides and example forms, and tried to do a search, but getting different answers... ... we are getting ready to submit our AOS package, and I am getting confused about how I write my name on the forms.

I am the USC wife, who is going to be changing her name very soon, but what name do I use on the forms? All of my documents are with my maiden name, but do I do ahead and use my married name? and sign that way too? Thanks for anyones help!!

OK and one more question?? He only has a Vaccine Card, is that good enough or will it be rejected because its not a I-693A? Where do you go to get that transfered if you need to?

Use what ever name that you wish on the forms. You are not applying for adjustment of status your husband is, and I assume he is not changing his name so this is not a problem.
