Just realised my Vacination Supplement form is only a copy

Oh no, with only one day left before the interview(DORA), arrived as a K-3 so therefore dont need the Medical just the Vaccination Supplement form I just realised that the Vacc Supplement form given to me reluctantly by the Surgeons office is only a copy and not in a sealed envelope, does anyone have experience with this?

If you mean the I-693A, ours was an original but it wasn't in a sealed envelope. It's the most unofficial looking form I have ever seen so I submitted the receipt where we paid for it. I thought if there was a question they could call the doc if they were so inclined.

If you mean the DS-3025, I can't recall if it was original or not. It was on yellow paper and it was also not sealed.

Probably the worst that will happen is you will be approved subject to submitting an original. But you might be ok.

Dont worry about it - I didn't submit a I693 - I just submitted a copy of the vac worksheet DS3025 and they don't look official at all) no one asked for anything at the interview relating to the vaccination. Good Luck

And if you mean the DS-3025 Vaccination Documentation Worksheet the original was in the plain brown envelope along with the other medical report forms and the rest of the case file from the consulate. It should now be in the case file at the District Office, waiting for your interview.

Actually I called a local designated doctor and just faxed a copy of what I had from the Canadian doctor and went in and picked it up, sighned my name and paid 10 dollars for it.. Thank goodness it was easy to get and cheap.
