Hello, we just called a Civil Surgeon (Dr. Cabral) in Elm City, NC, asking to get J's medical records transferred to the correct forms. His appointments person and a nurse of his are insisting on us making two visits (97 miles one way, and I have no time off from work!!) because they want her to take a TB test. Is this truly required for getting records transferred? They were being very ####### about it, and the other nearby office in Wilmington (88 miles away) has not returned my phone call after an hour. J hasn't had her TD shot yet, but is going to get that tomorrow morning hopefully at the hospital where I work. But Dr. Cabral's office doesn't know anything about her medical history, and all we want to have done is a simple transfer of records from one form to another...please give your thoughts on this, as this doesn't sound right...and taking off work like they are trying to force me to do is virtually impossible right now.

Call or look up Dr L E Arnold, she will do the whole thing by mail and for about $10-$20 in about 10 days! :thumbs:

I have a similar situation. My DS-3025 has exemption clause enabled (not applicable to K1) for vaccination part. So my vaccination entries aren't done. I have vaccination records up to date. Should I consult a civil surgeon or Dr. Arnold is going to help putting into I-693a the entries based on my vaccination records? Also should I mail her the original vaccination records or just faxing them is ok?

Make a copy of the DS 3025 for your records and mail the original to Dr Arnold, also put letter or note in it explaining what you need done along with the check for their fee, they will mail back to you the original DS 3025 along with a photocopy of the I-693a for your records, and also inside they will send a envelope that is sealed(do not open this) from Dr Arnold office(which inside it is your original I-693a she performed on transfer and signed off on)which is the envelope containing the I-693a that you will include in your AOS application when you mail in to lockbox in Chicago.

The DS-3025 is not applicable for K-1 because K-1 does not have a vaccination requirement...

Now, you are not applying for a K-1 but applying for adjustment of status.... these are different....

However, you certainly can do as others have suggested using the services of a Dr Arnold or the like....

I also used Dr L E Arnold. i called up the office, let a message. they call me back within a few hours. I mailed my wife's med. records with a $20 check and got everything back within 3-4 days. We filed for AOS with no problems.................yet! I was told that if we're missing anything, the office will call and let us know. Dr. Arnold's office was great!

FYI: Most of the Civil Surgeons just want to make money. I called a few offices in NYC: $250 on average with the usual 'It could be more if your wife needs additional shots'

Dr L E Arnold: $20 and that includes Priority Mail return.

We went to a local civil surgeon, since the clinic in Indonesia didn't give my wife an MMR shot. They were suppose to bill us, but that was back in December, and we havent recevied anything yet.

If you have all the required shots, then using Dr. Arnold is fine. If not, then you may need to find a local civil surgeon to give you the shot and fill out the form for you. You need to be very specfic that all you need is the vaccination supplement, otherwise they will give you the full medical.
