also on some forms they ask about State or provence in the philippines. should I leave this blank since the philippine definition of provence is different than ours?

"17. List all educational institutions you attend or have attended. Include vocational institutions but not elementary schools."

This directly indicates that you include everything except elementary schools.

 In addition write the name of the province where you come from. Philippines is divided into provinces and that applies in filling out the form. Im from the province of Zamboanga del Norte and I wrote it there.

Goodluck!!

list every school from HS on. if you don't know the address to a google, ask.com, & msn search + a Philippines specific version of the same searches http://www.google.com.ph/ http://ph.msn.com/ etc. you'd be suprised how much info you can really find if you put some serious time into it.( i found my SO's HS address & phone # (yearly enrollment of less than 100 students) off of a photo book some one that had gone to her HS posted 4 years ago).....but in the end if you can't find what you're looking for put "unknown"

use an extra piece of paper & attach it to the form. put a header on it "Question #17 form DS-157 for ...Name" but list everything from HS on. don't take any chances, if you can avoid them
