im gonna try this again since im getting mixed answers on this. does anyone know if the ds156 form for a child have to be with childs info, name, passport # and so on or is it just copies of the mothers information?>

Fill in duplicate and do not sign form DS-156 (2 identical copies)

If your unmarried children will apply with you, 2 identical unsigned copies of the DS-156 for each child.

The thing about it is that the DS-156 only asks for your Name, DOB, and place of birth. And it also has a section where you fill in the names of children that will be accompanying you. I think instead of printing 2 copies for your fiancee, print 4 copies if you bring 1 child, 6 copies for 2 children, and so on and so forth. But just to be safe you might as well make a completely separate DS-156 for each child also. And I do not see anywhere on the form where it asks for passport information. Just first, middle, and last name. Date of birth and place of birth. Then there is a section that only married couples would fill out, then there is a section about children that will accompany their parents. Nothing more than that but Juarez sent me a form that expired 5/31/2004 so maybe the updated version is different.

thanks for the reply gate, but the form that you mean is ds156k. the ds156 is the one that generates the bar code. thats the one that has passport info and so on and i have had different answers to this. so i guess my best bet is to just do 2 sets, 1 with mothers info and the other with the childs. i see that you are going to juarez at the same time. we will be there this monday for the medical exam and most likely once the results are in we should be ready for the interview thursday or friday. good luck to yall.

I answered to your same question not long ago :)

Believe me, DS-156 is the same form that is used when you file for other types of non-immigrant visas, like tourist visas, and every applicant needs one. Even if they are underage, they need to fill their form with their OWN info.

It does not make any sense to fill out the mother's information if the kids will need a separate visa (K-2) for each. DS-156K it's only for the mother, and of course, you need to write that the children will be acompanying her on the trip.

If you feel at more ease filling out 2 different versions, it's fine.. but trust me, each applicant needs one DS-156 with their own information.

Right. DS 156 is a visa application. Each person expecting a visa needs their own visa application.

The I-129f is not a visa application. It's a petition. Only one petition is needed for K1 and however many K2 children are needing visas.

1. After you finish the online DS-156 for yourself click CONTINUE, then PRINT (change # of copies to 2).

2. Once it prints, click BACK on your browser to go back to the online form, then click GENERATE FAMILY DUPLICATE. This fills in some common fields like (last name, address, etc) and you fill out the rest.

3. When done, again, click CONTINUE, then PRINT (2 copies).

Repeat for each child.
