What Order to put Latin Names on forms

Hi all,

My fiancee in Cali, like a typical Latin person, has several names. My question is, on the Fiancee Visa application forms, what order to put her names. Using the G-325A form for example, it has Family Name, First Name, Middle Name. Her name is Alicia Carolina Guerrero Duarte (first name, middle name, paternal name, maternal name). On the G-325A form how should she put her name? Thanks a bunch.

On last name, write Guerrero Duarte

First name Alicia

Middle Carolina

That's how I did it on ours.

Just as a side note, her mother's last name will never be used here in the States. So her name here will be Alicia Carolina Guerrero unless she changes the Guerrero to yours.
