DS 157 form?

My finace received form 157 enclosed in packet 3.5. I never heard anyone talking about this particular form. what is this for?

DS-157 is a supplemental form that goes along with DS-156. It is only filled out my male beneficiaries in a particular age range (16-45). I don't think it's talked about much on here because I would guess that the majority of beneficiaries are women, so they don't have to fill it out.

Sometimes they use this form for immigrants from predominantly mulsim nations.

1. In reftel the Department announced the introduction of a new form, Supplemental Nonimmigrant Visa Application (DS-157) to be submitted by all male nonimmigrant visa applicants between the ages of 16 and 45, regardless of nationality and regardless of where they apply (including CA/VO/P/D and USUN).
