Hello everyone!

I am kind of freaking out because I am still not clear about what this vaccination supplement is and if I need it. I just had my medical (not even 4 months ago) and our interview is in 2 weeks. Do I need it? I called my Dr. ( he was on the USCIS list of drs) and asked him if he had included this supplement but he was kind of clueless....

HELP!

When you had the medical for whatever visa you received, there are required vaccinations that must be completed before the AOS process completes. After the medical, you should have been given a form that either says all vaccinations are satified or what still needs to be completed. If all were completed, you should just bring a copy of that form to back up what is already in your file. If any are outstanding, you need to visit a civil surgeon, get the vaccinations you still need and will be given the supplemental form in a SEALED envelope. You will bring that with you to the AOS interview or send to the USCIS if you receive an RFE for the supplemental form.

NOA1's for AOS/EAD received Sept 20 MSC #

Biometrics completed Nov 5 2004

EAD Approval Dec 29 2004

AOS appointment letter received Feb 3 2005

AOS interview scheduled for March 9 2005

AOS interview mostly fine, just need I-693 supplement filled out by civil surgeon

Spend another $40 for I-693 form, sent to local USCIS office registered mail, arrived March 11 2005

AOS finally approved March 25th 2005

Green card arrives about 10 days later

!!Green card has incorrect middle name!! Thanks USCIS

Travel back to Denver, turn in card and I-90 to correct their mistake.

Application accepted, asked for I-551 stamp, they told me "We don't do stamps anymore"

NOA1 for I-90 received from NSC April 18 2005, fee waived

NSC appears to have stoped processing I-90's

Marilyn needs to travel back to the RP, call Senators office for help with green card or I-551 stamp

Travel back to Denver July 1, 2005 and received I-551 stamp in the passport.

Is this for your AOS interview?

The Vaccination Supplement is a medical form filled out be a U.S. civil surgeon showing you have received all the necessary vaccines. The official link is here and it's very clear at answering any questions you might have.

You can go ahead and get it done before the interview and bring it with you, or you can not get it done and hope they don't look for it at the interview. If they do look for it and you haven't got it, this simply means they won't approve you until they receive the Vaccination Supplemental form.

It's really your own call, but best of luck with the interview

I would get the vaccination supplement done ASAP if I were you. It's just another potential bump in the process you don't need if it becomes an issue.

10/25/04: Sent I-129F original submission (Nebraska SC)

8/1/05: K-1 visa approved!

12/3/05: My honey and I entered through Chicago O'Hare together. She is finally home with me!

I-485 AOS

12/21/05: Got married

12/22/05: Applied for SSN, had to do manual verification, 6-8 wks:(; completed I-693 vaccination supplement w/ local civial surgeon

1/09/06: Got SSN card in mail, this is fast!

1/20/06: Sent AOS, EAD and AP package to Chicago Lockbox

1/31/06: Received NOA1's for AOS, EAD & AP

2/3/06: Biometric appt letter arrived today (wow! that's fast!), scheduled for Wed. 2/22/06

2/22/06: Biometric done, but found out we had the wrong DOB filled out on her I-485, cannot be corrected at the time because her case isn't in the "system" yet. This is stressful.

2/25/06: I-485 touched

3/6/06: Infopass Appt in downtown Chicago to resolve the DOB issue. Had to send a letter to the processing center.

3/24/06: AP approved and sent in mail!

3/27/06: EAD touched.

3/28/06: EAD approved online

3/29/06: AP received in mail

4/29/06: AOS NOA2 received -- interview date June 28 2006. Online status shows still in process.

6/28/06: AOS INTERVIEW in downtown Chicago Sucess! Passport got stamped and Green Card is on its way!

7/8/06: Green Card arrived

I-751 Remove Condition

4/28/08: Sent I-751 package to NSC

4/30/08: NOA1 date

5/12/08: 1st NOA for biometric appt on 5/28/08

5/16/08: Another NOA for biometric appt on 6/5/08, we are confused

go to a civil surgeon, get a copy for your records and mail the sealed supplement in with your I-485.

To the OP. I/we did not receive any paper work following the medical examination done in-country, as is the case for many Beneficiaries depending on the country/consulate they're dealing with.

I our case my fiancee (now wife) was just given the sealed envelope, nothing more.

When I applied for AOS I included a letter in my package that described the circumstances and that if necessary we would do the supplement if we had a copy of the medical.

My case was transferred to CSC where we were subsequently approved with no interview.

So, it appears that not everyone requires a supplement. :yes:

Kaydee, I'm so thrilled for you that you slipped by without a problem.

However, there are enough examples on VJ of the supplement and even the medical being a problem that I don't believe it is responsible to suggest ignoring the CLEAR, CONCISE instructions on the interivew letter.

The instructions state "if not already submitted".........

Apparently there are many VJ'ers that do not require a supplemental if they were only given a sealed envelope and nothing more.

I did not "slip through" as I called attention specifically to this anomaly via a typed letter enclosed with my AOS.

There are many VJ'ers that do not have a record of the vaccinations given in-country and these kinds of threads uneccesarily cause them grief by implying that they need one. That is not responsible.

My contention is that they do not unless they recieved paperwork from the in-country physician showing short fall in their vaccinations requiring a follow up.

You say otherwise? Please demonstrate to me how you've come to this conclusion?

isn't that from supposed to be in the sealed envelope?

when I did my medical, I got 2 copies of each other than sealed one,

it had vac form, my hiv test result, and I-693 form.

of course that's because I couldn't find any info about my vac from my country which is sad it cost a lot more money.,

If you had your medical done with vaccinations in the US, then your vaccination supplement should have been included in the sealed envelope the doctor gave you, the one that you sent with your AOS application. Then you don't need it again, unless if the uscis interview letter specifically asks for it. If your vaccinations weren't completed in the US and you didn't show proof of vaccines to the civil surgeon in the US, then you need to get that vaccinations supplement from the civil surgeon.

But if you had the medical done overseas, as K adjustees would have had to, you wouldn't have been given an I-693A.

Like I said in the pinned post in the K-1 forum, VJers' experiences have been mixed on the I-693A issue--some folks have been RFE'd for it even if they included a DS-3025 with their AOS application, and some folks haven't. Because an RFE in AOS can be debilitating--it holds up EAD and AP processing, and it sets back the process at least a month in general--VJers have been advising each other to send in an I-693A just to be safe.

Sorry you're tired of my "rants".....

The simple fact is that I have read that thread in its entirety and it contains obvious inaccuracies that apparently you just seem to want to pertpetuate.

I stated what my sitiuaion is. You neglect the fact that not all beneficiaries are provided copies of their medical and my contention is that is key to whether or not a supplemental is required.

You have a different point of view? Fine!

Please don't wreck this thread with one of your rants. I really get tired of you wrecking threads when someone disagrees with you.
