the DS-157 is the form that is validated for visa payment. packet info is incorrect. you're fine.

At one time the DS-156 form was used for validation. Why they do not correct information is beyond my reasoning.

back in Dec 06 the packet also said the DS-156. at that time the ds-157 was being validated for visa payment. don't worry. you're fine

You'll be fine. It's the DS-157 form which will be validated by the bank not the DS-156. When I picked up my packet & checked the enclosed documents, I noticed that at the instruction sheet they erased the word DS-156 with white correction ink and wrote in black ink DS-157 & this form is the one validated by the bank. Likewise, I noticed the DS-Forms they sent in the packet expires in 2007 and the one I downloaded expires in 2010. I don't understand why they send instructions with error & expired forms  . Maybe we need to write a letter to them to remind them    Anyway, goodluck to you!

The DS-157 is the one validated by the bank. They probably do not update the documentation included in the packet...hell, the DS forms in the packet are all expired if you check...so don't forget to use the online forms...

And its much better to read their website for more updated info...

2-0. In favor of the boys.
