after my fiancee sent the packet 3 is there anythinng else that she must do?

according from her is okay to get her medical examination and the police clearance, is that right?

I don't know about Japan specifically, but in most (not all) countries, it is ok to get the medical done before any action from the embassy. If she said it's ok, I'm assuming she asked about it already. Police certificate can always be acquired at any time (as far as I know).

Aside from that, she just needs to be prepared for the interview. Does she have a completed I-134 from you? Evidence of relationship?

if she had all the documents needed in the packet3 checklist then all she has to do is sit and wait for packet4 to come to tell her when the interview is

Along with packet 3 from the embassy, they send a list of the doctors she can see to get her medical evaluation. She can not use her own doctor. They also should've have sent the form to show at the police headquarters for her prefecture for the police clearance. In many other countries, you can get these two things prior to the embassy sending anything, but not in Japan. The police will not do anything until they get the paper/form that the embassy sends out and the embassy does not list the doctors they approve of on the website or any where else.

So if she has already received and sent back packet 3, then she needs to complete the medical and police clearance before they will schedule her for an interview appointment. The embassy sends the instructions in English and Japanese so should be no confusion. Good luck!!
