vaccination supplement question...

Question for you guys....

When my fiance had his medical in edmonton canada, he was never given a copy of the Form DS-3025(vaccination documentation worksheet?)

All he has is his BC Health passport listing his mmr shot and his Alberta adult immunization card, listing his tetnus shot and the date given.

My question is...

Will we have trouble getting the vaccination info. put onto the vaccination suplement form I-693A Without the info. being on form DS-3025 first?

I have read on here some where that there is a CS doctor in Texas that will transfer info. onto I-693A For A Small fee. All we have to do is fax the info. to them. I just don't know if faxing the immunization card, and BC health passport (which is the only thing showing the immunizations given) will be sufficient.

Any info/imput would be much appreciated.

If you don't mind the thought of a possible RFE, just note in the cover letter that the immunizations were completed in the medical as part of the visa approval and is included in the documents surrendered at the POE. Quite a number of us have done it this way and were approved without a problem. The only time that I have seen a Canadian get an RFE is when they have lost the original medical or it is more than a year old. You can read the pinned I-693a FAQ's in the AOS forum for the results of the lost medical cases. Another option would be to contact the panel physician to request a copy of the vaccination worksheet and/or medical. It's worked for some. If you haven't done so already, give this thread a read.

I also had my medical done in Edmonton and had the exact same question. I got the vaccination supplement from a Doctor here in Cali. It costed $15. Also, I had to pay $35 to be tested for chicken pox immunity. I would shop around with the doctors because some charge up to $150! Make sure you tell them that you do no need the whole medical and only the supplement.

David used the doctor in Texas for the vaccination supplement. It cost us $20, and all they requested was his vaccination record and some ID... He got the DS-3025 from Dr. Chana though so we sent that anyway. The only problem with normal vaccination records is they don't include the chicken pox immunity, unless yours have proof of it somewhere. If it doesn't show that you'll need a blood test as someone else just said to confirm that you are immune.
