DS-156 DS-157 and DS-156K

Question about DS-156 DS-157 and DS-156K forms.

My fiance has mentioned to me that there is one form that you have to pay the bank. I do not know what form this is.

She mentions that its the DS-156 DS-157 and DS-156K forms.

But I know from Packet #4 that DS-156 DS-157 and DS-156K forms are free. Anyone can just download them and you can fill it up yourself.

Does anyone know what form that is that you have to pay and get from the bank?

I think my fiance is just a little anxious and she maybe is talking about the Visa application fee.

Yes you are right, your fiancee is probabaly referring to the form that needs to be validated when sher will pay the visa application.Goodluck!!

I filled up all my forms online.

It is the DS 157 that will be validated by the bank when you pay the Visa application fee. The bank gave me a blank DS 157 with the validation in the back, then attached it to my filled up DS 157 form.

So the DS 157 can either be downloaded and you can validated the form the bank if you pay the fee.

Or you can purchase a blank DS 157 from the bank that is already validated.

First and foremost, the forms are free. They can be downloaded from this website in the guides section, from the Manila Embassy website, they will be sent to your fiancée in packet 4, and/or the beneficiary can pick them up from St. Luke's.

What happens is that the bank validates the back (rear) of a blank DS 157 upon payment of the visa application fee (so it appears as if you are paying for the form, but you are not). Some banks only validate blank DS 157 forms they have in house. Others will validate a completed DS 157 form that you provide, but that is rare. So, I recommend calling the bank beforehand to inquire if they are willing to validate a completed DS-157 form.

If the bank(s) will not validate your completed form, you may either type in the information on the DS 157 post payment of the visa application fee or submit your completed DS 157 and provide the blank validated DS 157 as proof of payment. As backup, make a copy of the receipt and bring that to the visa interview as well (just in case because the folks just happened to ask my wife for it).

For me, I'm always doing research on the USEM Guide. However, my fiance gets a little anxious and starts worrying about things.
