The pinned post on the vaccination supplemental reads:

Q: Do I need to complete another full medical to adjust status?

Per Section 1245.5 in the Code of Federal Regulations, a medical examination shall not be required of an applicant for adjustment of status who entered the United States as a non-immigrant spouse, fiancée, or fiancée of a United States citizen or the child of such an alien as defined in section 101(a)(15)(K) of the Act and §214.2(k) of 8 CFR chapter I if the applicant was medically examined prior to, and as a condition of, the issuance of the non-immigrant visa; provided that the medical examination occurred not more than 1 year prior to the date of application for adjustment of status.

My wife had her physical more than a year ago, and we have not done an AOS for multiple reasons. Bottom line, now I have to have a new physical done? She does have a vaccination card that was given to her after her physical in Nicaragua as part of the K-1 process, but I doubt that makes any difference.

Please advise. Any help is greatly appreciated.

Yes a full I-693 will be needed to adjust status.

Bring the vaccination card, she may need a booster for MMR, or can be tested for immunity by titer test.

Yes you will need a new medical... bring the vaccination records and they will prepare the supplemement.

So we will need forms I-693a and DS-3025?

If you have the DS-3025 bring that with you... The CS will prepare the I-693 and the vaccination supplement...(often referred to as the I-693a)

Actually I think I should be a little clearer. All she has is a vaccination card that she was given after her physical in Nicaragua. It is not on any sort of U.S. form or anything. I think I should just take a blank ds-3025 and I-693 and I-693(a) and see what the physician tells me. Any thoughts?

My Wife got a vaccination book that listed what vaccinations she had, and the US Civil Surgeon transfered this record to the supplement (I-693A)

You do not need to bring any forms with you to the Civil Surgeon, they will have all that is needed... Bring whatever record you have for vaccinations (translated into english if needed).
