suggestion advice regarding accident

im compling all the necessary docs needed for pre-interview :

We were in the middle of class discussion that time, when my high school teacher in trigonometry asked a favor to open the glass window from the corridor. Since I was the nearest one, I voluntarily opened the glass window. Unfortunately upon pushing the window to open, it broke into pieces and caused laceration on my right wrist and hand. My teacher and my classmate rushed me to the school clinic to be given first aid.

i duno if i need more evidence on this ( caused laceration on right hand and wrist )

- incident report signed by princial, school nurse, directress with school seal

- notarized incident report by a notarian lawyer

what else do i need?

2.) the family pediatrician already flew to the states, what can i do to get the vaccine records?

you don't need to bring your vaccination record if it can't be retrieved. They didn't ask for it when i had my medical. Just asked me if I have a chickenpox scar and the BCG scar (which is usually found on your right deltoid area).

if you can provide documentation that it was an accident, that will be better :) the doctor might automatically think that was a suicide attempt so if she asks you about past medical history, be sure to bring that up and provide documentation :)
