Doctor for I-693A in Minnesota. Help!

My wife has entered the US with her K3 visa and we are filling out the AOS forms.

We will really appreciate some help.

As we know she needs just I-693A (http://www.cdc.gov/NCIDOD/DQ/pdf/ti-03/appdx-a_693vacc.pdf - is this it?)

form but we called many clinics, nobody knows about this form or just talk about full examination again.

Please if anyone knows doctor/clinic in Minnesota (twin cities) who can do that we will really appreciate.

I posted my documents and 20$ fee to Dr Arnlod office in Austin, TX. I think this is the good idea rather than calling 100 other doctors..

Yep I too went over Dr.Arnold.

She is in Texas,you dont have to live in Texas though to send her the vaccination documentation worksheet. It can all be mailed. 20 Dollars really is afforable.

I sent my vaccination records to Dr. Arnold last friday with $10 cheque only. The secretry called today informing that they sent it out yesterday. I wonder why some had paid $20?

Dr. Arnold wont be able to help if you still need to complete some vaccinations.

If thats the case, look for the University Community Health Care center.

wow! $20 is a really good option. We went to the New Americans community center in St. Paul on University ave and it was $100 but we got the form very quick.
