name on vaccination supp form

My married last name is on vaccination supplement form when we filed it. Think there will be no problem with that since you'll be attaching a marriage certificate on your packet

Does it make any problem not with my marrried name? Coz i didnt send my marriage certificate to Dr Arnold.

Old name or new name it really does not matter because most likely your medical is not in your married name and they process it... as long as they can tell it is yours they will proceed.

I put my married last name and nobody even asked my marriage certificate + they saw my maiden name in passport as they needed copy of my K-1...

I agrea with fawguy , married name or maiden doesnt mather as long as they know its you ,

( and they will know its you since they will have to go back in your k1 file to retrieve the medical you had abord , as the medical is in the same folder when we file for I 129f and the K1 , I know cose I saw all my files in the folder at the border when I activated the k1 and handed them the brown envelope )

I filed married name ( and didnt include a marriage certificate , ( you will be fine )
