#30 it asked. Have you ever been issued a U.S. Visa?

then it asked enter additional visa issuance..

My question is: Do I put the Issued and expiration date? Also I can't remember the first visa issuance... What if I can't remember it? Please help..

Have you? If so, just put whatever you can remember. For dates that are forgotten, you can just use months and year. Don't you have any records kept somewhere?

if you've been issued a visa before, all the details (issuance, country of issuance, expiry) should be on your passport (the new or recent one). Look for the old Visa as well as the arrival/departure stamps located on one of the pages...

I was issued a visa before (B1) and just had to look for the details in my old passport...
