Hello, i am trying to gather documents for AOS. I have confusion on vaccination supplement form I-693A. I only found form I-693 not 693A. Pls tell me which one should I file.

Second question is should i write my married name on I-485 form?

Can I send AOS without SSN filling on the forms?

You should have received a copy of your vaccination record from the doctor you went to in your home country.

You should find a Civil Surgeon approved by USCIS (a list of doctors is in the guide/link above) and ask them to transcribe the vaccination record onto a new I-693A and you will include the new I-694A in your I-485 application. The doctor will probably charge you something (ours charged us $25.) If the doctor insists you need a new I-693 and not just I-693 don't listen and show him in the instructions where it says you just need the supplement.

If your medical from your home country is not yet a year old (just checked your timeline, and it can't be more than a year old ;)) you do not need to worry about I-693 without the A, unless they issue you an RFE because they lost it, and in most cases they don't lose it thankfully. So don't worry about that unless you have to when the time comes! :thumbs:

No, you just need I-693A, the vaccination supplement. Don't worry about the I-693!

(If everything was done correctly) - after you gave the medical to the officials at the Consulate where you had your interview, they put it in a sealed envelope that they sent you in the mail along with your K1 visa. When you entered the U.S. the POE officer asked you for the sealed envelope. The I-693 was in there and hopefully was forwarded to where it needed to go.

Sorry I missed your question about the married name! Lona is right, if you took your husband's name then use it when filling out your forms.

And you don't need to put a SSN on any of the forms. We didn't. We got my SSN after we filed, but before my I-94 expired 3 months after my entry into the U.S. If your I-94 isn't expired yet you can apply for a SSN if you want to and haven't already but once it's expired you'll have to wait for your EAD to get a SSN.

And you don't need to put a SSN on any of the forms. We didn't. We got my SSN after we filed, but before my I-94 expired 3 months after my entry into the U.S. If your I-94 isn't expired yet you can apply for a SSN if you want to and haven't already but once it's expired you'll have to wait for your EAD to get a SSN.

Actually you need to apply at least two weeks prior to the expiration of the I-94. Otherwise you will be denied.

Yes I do have a copy of form DS-3025 given by the US embessy certifed doctor and copy of vaccine report. One vaccine TD was done after interview as suggested by doctor.

SO how do i get a vaccine supplement I-693A.

Locate a Civil Surgeon and ask the Civil Surgeon to transcribe your DS-3025 to an I-693A. That's it!! Be prepared it will cost some money and they might tell you you need the I-693. But you don't.

Did you get a chance to check the link about where to find the civil surgeons in your area?

Make sure you call around for pricing, it can vary by a tremendous amount. We paid $70 to have a tetanus shot and the CS fill out the I-693A and seal it in an 'official' envelope. Others have paid less but a lot paid more.

WOW That's a lot for a Tetanus shot... I just paid $10 for mine when I had it done in Canada! Now $10 doesn't seem so bad!

Nice! To the OP you can try that if you want as well, although many of us didn't because we read on here that some have been successful and some have received RFE's because they sent the DS-3025 instead of the I-693A and we didn't want to risk an RFE. At least $25 wasn't really a big fee for us to pay for my vaccination record to be transcribed - compared to a $300 medical anyways.
