I am filling out i-485 for my wife(she is on k3 visa). i need help with some of the questions....

1) Place of Last Entry Into the United States (City/State)

from google JFK airport is in JAMAICA, NY. is that the correct answer?

2) Nonimmigrant Visa Number

is this same as control number or number in red at the bottom of visa stamp?

3) Expires on (mm/dd/yyyy)

is this k3 visa expire date?

4) Part 2. Application type, I am applying for an adjustment to permanent resident status because:

for k3 visa, i should select option a, right? (my i130 is approved)

is this the "A" number from i129f noa2?

2) my wife has k3 visa. she has her medical done in india to get k3 visa. only thing we has is vaccination documentation worksheet and x-ray.

do i need to fill out i693 form?

1) Use the A# that is on the visa or maybe written on the I-94

2) You cannot fill out an I-693 as it is the full medical examination performed by a Civil Surgeon. The I-693 is only required if the foreign medical was performed more than 1 year prior to the AOS application otherwise a vaccination supplement is all that is required

1) Hmmm...It was on my wife's I-94... I would then use the A number from the petition approval NOA. Are the "A" numbers on the I-130 approval and the I-129f approval different?

2) Some people have been successful supplying only the DS-3025, others have taken there DS-3025 and paid to have a civil surgeon transcribe to a vaccination supplement
