Yes bec. it helps (easy to find) the examiner to check all your documents, It's nice to see if well organize too! There's a sample here click on example forms on top! I agree with the folks, it helps YOU know exactly what you're putting in the package, and it makes the dude (or dudette) who receives it much easier than to wade through kilos of photocopies with no index. Remember to keep a copy of it, along with everything you send.
