On the cf-3299 it asks:

"5. Importer's Port of Arrival"

"6. Name of arriving vessel carrier and flight/train"

I am taking these to mean when I enter the country, since I will not know when my unaccompanied goods enter. I am planning to ship them UPS...so would I put for #5 - Chicago, IL and for #6 the airline + flight number? A little confused what it is asking for...

Yes you are the importer so put your travel details, that is what I did.
