Timeline

My then fiance had to take a medical exam prior being interviewed in Ciudad Juarez for her K1 Fiance visa. The VJ guide suggested that if such is the case, we'd only need to file the vaccination supplent I-693A. The guide did not include a link to this form nor does the USCIS site have this form.

The only medical document we were allowed to keep after the K1 interview was a sparsely populated "Vaccination Document Worksheet" ...

Potentially really stupid question then ... do we need to have a medical examination done prior to submitting the I-485 application package?? Thanks for bearing with me ... :unsure:

a civil surgeon can transcribe the panel physcians worksheet over onto the I-693A vaccination supplement, if all vacs are complete you can send it to Dr Arnold in texas and she will do it for $20.00.

I havent got the address saved but there are a tonne of threads on here about her.

Unfortunately, I think we will need to get my wife's vaccinations updated. Since we had gone through the K1, vaccinations were not required. Can anyone else that went through the K1 confirm this?

Well I had all my shots compleate.

However some shots are not age appropiate,just depends on the age.

If your vaccinations were complete at the time of the foreign medical then you can submit the DS-3025 with your I-485 application. If you updated your vaccinations subsequent to the foreign medical then you need to take you DS-3025 plus the record of the new vaccinations to a civil surgeon who will transcribe to a new vaccinations supplement (somtimes here called the I-693A)

It's still a good idea to have the vaccines transcribed to I693A - even if the Panel Physician validated you at your medical - since that is the form that many adjudicating officers prefer to see.

And if you are one of the fortunate to have a personal interview you will most likely will have the time to get it done on the interim...

Thats great,it really does make a differance to make a few phonecalls.

Dr Arnold used to do it for 10 Dollars,it has gone up in price now though.

I don't know how to quote from more than one post in the same reply so I copied parts of two of your posts into the same quote above ;)

Anyway... I've adjusted from K-1 (check my signature). Medical is valid for 12 months. If you are filing for AOS within that time you don't need to have another medical.

Vaccinations are not REQUIRED for K-1 but many consulates do the whole thing. Sydney does. Your wife's consulate does. The "Vaccination Document Worksheet" your wife was given after the medical is all you need. It probably has a few "checks" to indicate that vaccine is "not age appropriate" or date when it was given, or date the lab tests confirming immunisation were done. Don't worry - it is "a sparsely populated" worksheet. Just have it transcribed as other posters advised.

I even went as far as filing only this copy from the medical, and was never asked to supply an official I-693A. But I guess I was just lucky. Don't try if you don't mind possible rfe delay :)

PS: Have you checked the bottom left corner of the vaccine worksheet? Under "2.Results" - it will tell you if it is complete or not.

What concerns me is that the document I posted in the first post was the actual document with information blurred out - it has nothing filled out ... just "FIANCE" in bold. Neither are any of the boxes checked. I'm thinking the simple answer is that we need to her vaccinations updated as we opted against it in Ciudad Juarez for the K1 interview. :(
