Police report UK

I am trying to fill out my police report form and it is asking for the nature of the information that i am looking for does anyone know what to put in there ???

what choices does it offer up? I did mine a while ago but if you give the choices I should be able to remember

its asking for crime number or incident numbers victim witness etc

This is wierd the form I got to fill in was Application for "prosecutions, Convictions and Cautions and doesnt look like the form on this link and today I recieved a letter from the Subject Access Office, New Scotland Yard, London headed: "Section 7, Data Protection Act 1998 - Subject Access" saying from the personal details supplied there is no information held aboput me in the Person Record category of the Police Nationa Computer. Is this correct?? does anyone know?

Ditto this GMP form is nothing like the one I had from the Met Police...

This is the same as the one I have my hubby has the letter but also the report that shows he was a bit of an idiot when he was young

All I did was phone the local police station and asked for the Scotland yard address and wrote to them telling them I wanted our police records for immigration purposes and included a £10.00 fee, voila, all received less than 1 month later

Well thats what I did went to my local police station told them i needed this form for our visa application and she called it the "Subject Access form" and I said yes as it sounded familiar and she gave me the form as I say doesnt look like the form linked here all it has on it is my name details and my resent address and to list any previous addressess if i lived at current address 10 years or less. As I have lived in current address longer than 10 years I didnt include any other previous addresses. I just kind afreaked as the form linked looked different to mine. But i got a letter today from scotland Yard saying im not on the computer so I think it should be ok
