UK Passport name change questions to any ladies out there

Hey all, does any Brit know the answers to the following on the C1 passport name change form:

I am sending off my C1 to Washington to have my new married name on my passport....

On question 1b I hate to say it, but I have no idea which to tick!,

I was born in the UK so does that mean I was naturalised or registered so i do tick yes? At the moment I ticked no, as I think that as I was born there I have not applied to be registered. Arghhhh, anyone know?

On question 9, the declaration..... it says (3) I am today in the country of application...... I guess that is ok, as this British form is going to Washington, DC, so that would be the country of application, right?

Do you have to send in a marriage cert if you are name changing?

It seems odd filling out a British application to send to DC, asking for your postcode, I have just put my american address in with zip...

Sorry to sound stupid, I have read the notes but still can't figure it out, I have been reading to much immigration stuff tonight and my head is full! :blink:

I have to have my passport for my AOS interview and can't risk DC sending it back to me cause I filled in the form wrong. :blush:

Met my partner - Jan 22nd 05
Got engaged - May 6th 06

Sent I-129F to Nebreska - Feb 21st -07
NOA1 - Feb 28th -07
NOA2 - May 22nd -07

Applied for my police record - June 4th -07
Pack 3 arrived!!! - June 21st -07
Police record(37 days)! - July 11th -07
Posted pack 3 back via signed for, next day delivery - July 25th -07
Medical - August 2nd -07
Apparently pack 3 was today signed for after going into a PO Box! - Aug 3rd -07 (9 days late)

So far 21 weeks


Pack 4 turned up! Interview end of August! - August 9th 07

Interview - 31st Aug 07 -Approved!!
27 weeks and 3 days from start to interview

A. Tick No

A. Yes you are in the country of issue

Do you have to send in a marriage cert if you are name changing?

A. Yes you need to send a Original copy of your Marriage certificate. Not a photocopy..

A. Thats all you need to put
