london medical exam question

hi guys

getting everything together for my interview and medical exam - i called up the kensington docs today and made an appointment, but they said i needed to bring along a questionairre that was supposed to be 'part of the pack, with the maps' - now i haven't received any maps, or a questionairre - can someone enlighten me about this? all i have is a letter giving the date and time of interview, a list of the vaccinations i need and a couple of other sheets, stuff like what papers i need to bring with me. will the 'pack' follow on? can someone help?

If I remember correctly the Questionaire is on the back of the sheet with the list of Vaccinations and prices on.

it's not on the back of ours! wah! should we contact the embassy in london about it?

There is also a scan of a medical questionnaire on VJ under packet 4 (for some reason). I notice though this is IV21a and it's rather dated. Anyway it is here.

Those who have already gone through the medical may be able to help further. It's my understanding though that they ask you to fill out the exact same questionnaire at the medical. Someone correct me please if this is incorrect.

That's interesting - thanks for the info. the woman i spoke to at knightsbridge this morning was a bit unfriendly and brisk with me and didn't seem to care that i didn't have the forms - just told me to look for them because i must have them... but needless to say i don't. can someone who's had the medical at london already confirm if i need the questionairre beforehand or if i can just fill it in on the day? want everything to go smoothly! thanks!

I think I got mine from the travel.state.gov website. Can't remember what the address is though :-(

This address is on the appointment letter NVC sent.
