So, we got an RFE today for the immunization form. I called the CSC and the lady said that they likely had received it, but it was filled out improperly by the surgeon. Can anyone see what's wrong with our form? I have a photocopy of it.

DT/DTP - Not age appropriate

TD - 9/19/07

Polio - Not age appropriate

MMR - 7/17/07 and 9/19/07

Hib - Not age appropriate

Hepatitis B - Not age appropriate

Varicella - 9/19/07 and 10/17/07

Pneumococcal - Not age appropriate

Influenza - Not age appropriate

He checked the "Applicant eligible for blanket waivers" box, stamped and signed his name, and dated the form. However, they didn't fill out my husband's passport number or his country, and his first name and middle initial are in the wrong spots. The lady on the phone didn't think that they would give me an RFE for those things being blank, but who knows?

Please let me know if you guys have any suggestions! I don't really want to pay $40 to go back to the civil surgeon if this is his fault in the first place. I might try just filling out the country and passport info on our photocopy and send that in (both options were suggested by the lady on the phone). What are the consequences if I guess incorrectly. Denial and another $1000 down the drain? Ahh! Maybe I will just go back to him...

Have you already received the RFE in its paper form? It should state exactly what they find 'incorrect" about the immunization form. If it is unclear....I would contact the Civil Surgeon, point out that some spaces were left blank on the form and ask if he can re-do the form(hopefully at no charge) and fill in the missing spaces. Hopefully that will work. :)

I would not fill in the photocopy and send that....that form is to be filled out by a Civil Surgeon...and usually they present it to you in a sealed envelope, so that no alterations can be made to it. (I'm actually surprised you have a copy of the immunizaiton form. Did you specifically request a copy from the Civil Surgeon? We didn't request one, nor did he offer one, just gave us one sealed envelope, which we included in our AOS package.)

tkae it back to your doc and show how he has incorrectly filled it... i think all responses for the med have to be in a sealed env, so even if you correct it and send ti back, you will get denied...

i myself got a medical REF last week, this was the SECOND time i have had to do the medical, the first time they messed it up because i fail the TB test, so i did a chest XRAY but the doc marked NOT TESTED, NORMAL CHEST...., and for TB he marked FAILED.... so they did a RFE for the chest xray

the second time, he filled evyerhing correct, but didnt test me for TB coz he knew i wld fail it, but the moron marked NOT TESTED for TB..

so now i have a RFE saying that everyone must be tested for TB skin test..... smart huh?

anyway., now they cant find my TET shot data., so im going to go in., spend a #### load of money for no reason and get it taken care off.., its worth the PASS GO and collect $200.. rather than the go directly to jail....

9/13/2006 - interview. waiting for bkground, no approve.deny case will be reviewed as my case in more complicated., i1131, i485 sepereated from file, claimed that they will be sent for processing

9/22/2006 - called national customer support., been 90 days, no interim ead (i knew i wldnt have it yet) was told to go to santa ana

9/26/2006 - office claims file is floating around somewhere in the mail room, they will find my work permit app and travel doc app and send it for processing

9/26/2006 - drive to LA to double check what they said, office at LA claims that santa ana sent work p, travel to NBC

9/27/2006 - EAD, Travel docs touched

9/28/2006 - EAD, Travel docs tocuhed

9/29/2006 - :( nothing touched, no letters no emails :(:( ---- waiting., will send letter to congressman monday

10/2/2006 - Travel Docs Touched, nothing new on EAD....Wife sent letter to congressman

10/2/2006 - Got Travel Doc in the mail

10/2/2006 9:30 PM PST, 5 email ead approved, will mail me new card

10/6/2006 10:30 AM PST, got 3 emails saying that they have mailed out an approval letter for my EAD... hmmm wheres the damn EAD,

10/7/2006 - Got EAD with a TYPO in a#!!!! in the mail

Hopefully your CS will understand that there was apparently an error and fix it for you at no charge.

The I-693A does not have to be sealed from the CS. The I-693 is supposed to be...

The I-693A does not have to be sealed from the CS. The I-693 is supposed to be...

The I-693A I sent was sealed... I believe all medical reports/exams have to be sealed.

Where does it say you don't have to seal it?

Thanks for the advice, you guys. I think the CS must have given us the copy of the form, because I don't remember making a photocopy. I'll go and see if they'll fill it out again for free, and if not, just pay the money. Thanks again!

hi again, so i went to pick up me redone medical,.. since they had messed up on the skin test...

i walk in tell them that im here to pick it up and get my tet shot, they charge me $47!!! for a tet shot... then they ask me to look over the medical form... there is a mistake they selected NO reaction for tb skin and normal for chest, i point it out they fix it., i get my tet shot, they give me sealed env, i say hopefully i wont be back for the 3rd time!!!! i leave..

on my way back i am driving and looking for the 693A, i see that they have marked , in the results section that i can apply for blanked., and that i can apply for blanked choose to deny based on religions (not sure on the exact verbage)... but then they SKIP the checkbox that i have met all requirements., and they check the one that says Does not meet requirements., i call them back., they say they made a mistake., so i flip the car go back.... and they fix it., they were supposed to mark just the one that says met requirements!!

so... what i am trying to say... does the one he marked., in section 3,. results., really apply to you?? or was he supposed to mark that he met all vaccination requiremetns?

I think that was the correct box to check, given that he hasn't received some of those vaccinations due to age. But I don't know. You would think these civil surgeons would have enough experience doing all this stuff that they wouldn't mess up so much. I'm sorry it's been such a dilemma for you, too. :(
