Depends on the state. For NY, you need to mark your dependent as NRA (Non-Resident Alien), but for CT, ITIN from Federal is needed.

I know this is late, but I process W7 applications.

There are only 13 types of documents the IRS will accept.

A US notary, or US State Department Employee may, certify/notarize the documents submitted.

Documents that are faxed to a US notary and submitted will be rejected.

Original Documents are a pain because some poor sap has to mail it back to you.

If you are in a foreign country go to the US Consulate and ask them to Notarize/certify the documents.

If you choose to submit a Notarized Copy of your passport, you ONLY need to submit the pages with your name, picture and issue date. You do not need to send in a copy of your visa IF YOU submit a Passport. Also, you do not need to copy every single page of your passport.

A passport is the ONLY document that can submitted by itself, otherwise you need TWO documents. You do not need to submit your I-94.
