looks good to me , but my advice is dont send any original certificates its not required to send the originals , copies are good enough , ( birth or marriage )

you wont get them back , you can show them if an interview is needed , good luck

Do NOT send originals.

Sending copy of marriage certificate is good enough you don't have to send marriage license..In the state of FL; the marriage license becomes your marriage certificate when you get married!

No need to send TD shot record as long as the vaccination supplement has that information.

Write your full name and A# on the back of all your photos..Otherwise, everything looks good :thumbs:

No. The filing fee now covers the EAD and AP.

Jen, looks good but as the others have said, send photocopies only. Take the originals with you to the interview if you have one.

Explanation for Eligibility on a separate sheet...what do we have to put for that ? I filled out the box on the form should I instead right a letter saying in case there is an emergency in canada I can see my family or something?
