Can someone help me with a few questions on the AOS forums?

I am a little confused about what last name to use on the documents. We are now married, do I use my last name on documents for my wife or her divorced last name that she had before we got married. I know women that marry in the USA that are citizens need to change names but what about my Russian wife?

Also she has a 15 year old son that I have to file for also. What documents do I need for her son? I need two G-325a, I-485, I-131, I-765, but do I need 2 sets of I-864? If anyone can send a link to examples of doc’s and what they look like that would help.

Also how long are the travel doc’s good for after you get them and what do you but for travel dates if you don’t know when you might travel?

I am also confused about the I-693a and DS-3025 we have copys of the DS-3025 but that is it. Do we need the I-693a or did they send it with the brown closed packet on entry. Can i send just the DS-3025?

Your wife doesn't HAVE to change her name, if she wishes to...now is the time to start using the new name...they also have a field for the former last name on the forms.

The son's is an entirely separate AOS pkg (although can be mailed together)..same sets of forms...you will need copies of your marriage cert, the I-864 you are putting in her pkg and anything else they ask for.

I am german citizen ( was canadian LPR ) am kinda in the same boat as your wife (previously divorced and used my ex"s name till I got married to my babe in dec )

my advice is , if your wife took your last name when you guys got married ,

I would put your last name on the application , as that is the name she will use in the future ,

yes it will be diferant then her passport name , but if you deside to travel once you have the AP , you can show prove of name change by showing a copy of the marriage cert ,

am not sure if russia carries the same law as germany ,

but in my case to get the name in passport changed after marriage ,

I will have to go to german consulate apply to have the marriage registerd in germany ( do a name declaration ),

once its registerd in germany then go back to the consulate and aply for new passport ,

you could call the russian consulate or look it up on line ,

as my passport is still valide till 2015 I,m in no rush to get the name changed till I have my GC ,

once I have the I 131 aproved will just bring the marriage cert when I travel ,

and do the name delaration once I have the GC ,

hope this aint to complicated to understand , and good luck

Here's what I did: I used my married name in all correspondence with Montreal, and they responded using that name. HOWEVER, when time came for the visa and then the green card being issued; they used my maiden name (since that's the name on my passport). So basically she can use whichever name she decides on, but the green card will match her passport.

don't know about the child, sorry.

I think it depends only on the name she uses on the forms, right?........all my correspondance and upcoming GC will be in my married name, despite the fact that my Canadian passport is in my maiden name. When i travel, I will carry my marriage cert until it comes time to change the passport in a few years

Len, do DCF's file an I-485 following entry to the US or is the GC generated automatically?

And, yes, I entered my married name on the I-485 also. Udella, when you travel, book your airline tickets in the name that matches your passport. Most airlines won't accept the marriage certificate as verification that you are one and the same person.

Read the I-485 instructions..On page 4 under section 8, it states you need a vaccination supplement for K-1. so better to get the I-693A filled out by a civil surgeon

K-1's don't need to send in the I-693a if the vaccinations were completed prior to visa issuance. See pinned topic. As fwaguy said, you are fine with just the DS-3025 if the vaccinations are complete.

I dont know if they were she had a US embasy doctor do the med. in Kiev but her DS-3025 dosnt show anything on it. could it be the I-693 was in her packet on arival. Also I dont see a I-693a for downloads just a I-693

Thank you all for the help, So it looks to me that I have to do two I-864 docs. I quess if asked if i sponsered anyone else i put yes and use his mothers I-864.

Yes but they should have diffrant information right you would have put your daughter as the main person on hers right.

Maybe ask her if she received all her vaccinations. Are you sure the DS-3025 is completely blank? If it is then she will need to see a Civil Surgeon to have her vaccinations updated. They will have the form you require. Don't let them talk you into a whole new medical. If the current medical is less than a year old, you don't require another one.

The I-693 is the medical and was in her packet at the POE. The Civil Surgeon will have the I-693a.
