Hi!

My wife arrived from the philippines with only an almost blank DS-3025 and nothing else from her medical exam. She says they told her she needs two additional vaccinations but they didn't document anything on the DS-3025.

It has her name and identifying information and says, "NOTE: The applicant prefers to vaccinated in the United States."

That's it. No information about previous vaccinations received at all.

We are getting ready to send in her AoS papers and don't know what to do about the DS-3025 and/or I-693a.

Anybody know what to do?

Visit a USCIS Civil Surgeon, and ask for them to complete only the vaccination supplement. They will also complete all of the necessary vaccinations and provide you a sealed I-693A.

Maybe, its up to the doctor.

There are some shots that you will need records showing that its been done more than once.

Although USCIS states that you need to get the missing vaccinations and the I-693a, many of us have found that you can just send the DS-3025 with your AOS application and they approve it anyway.

My wife was clearly missing vaccinations on her DS-3025, but we had heard that they were approving them with only DS-3025 (even with missing vaccinations), we sent ours in and it was approved with no RFE and no interview. Seems strange but it's true.

So if you want to save some money, just send it in with the DS-3025. Worst case is you'll get an RFE for the vaccinations and will have a short delay, if not you'll save the $$$.

If she did have some of the vaccinations before, she could just have titers done to see what she still needs rather than getting them all done again. If the titers show immunity, they can record that on the vaccination record as an explanation of why those particular vaccinations were not appropriate. I'm not sure if that would be worth the extra trouble, but its an option.

ROC Journey:

01/19/2010 - Mailed ROC paperwork to Vermont Service Center

01/21/2010 - ROC package arrived at VSC

01/26/2010 - Check cashed

01/28/2010 - Received NOA, GC extended for 1 year

02/25/2010 - Biometrics taken

04/23/2010 - Conditions lifted! :)

05/01/2010 - Ten-year GC received...on hubby's birthday! Yay!

Here in VJ, some people said that they got her vaccination in a local hospital here in US for free. Maybe you can try this choice. I think if you don't have any records like proof that you already have your vaccination you must get all vaccines again at least the imigration doctor makes you a test for check your antibodies... Anyway it would cost more money...

If she got a shot at SLEC they will probably state what it is, like if it is MMR or Varicella vaccines since TD shot is not available in PI. One of my friend has a blank vaccination worksheet coz she is pregnant and she prefers to have her shots here in US. You should get these shots on USCIS civil surgeon nearest you. You can also go to health clinic to get the shots and then ask a civil surgeon to transcribed it on I-163A this will cost u less. Goodluck!!!
