hi guys,

first things first, i just received my packet 4 today from USEM.

but i really need help on some of my questions, i apologize too if my questions more than one or two.

as i read the instructions, it says that on DS156 and DS157, each applicant must submit 2 completed copies of the form and in DS156K each applicant must submit 1 copy of the form.

I am the primary and we have a daughter which is K2 and shes USC(holding US passport).

My questions are:

1. are both of us will fillup the form or just me?

2. will i use the forms i received today?

3.I read from this site that others downloaded forms online, so do i have to that also?

4. or is there any other forms i have to submit other than this forms i received from USEM?

5. my daughter do not have to go medical exam becuase shes USC, am i right?

6.do i need to fillup the forms first before i bring it to the bank, pay and validate?

7.which form is to be validated, the DS156 or DS157? blank or already filled up?

i know these probably been ask before, but i can't retrieve what the date was.

so pls help.. :help::(
