Hello all,

Anyone ever fill out I-90 (Application for Replacement PR card)? There's some questions there that I'm not sure if my answer is right, maybe you can give me your opinion...

Part 3: Processing Information.

- Question on the form: City of Residence where you applied for an Immigrant Visa or Adjustment Status

My question: Is this means current city where I live in (my mailing address) or is it the city where I applied for K-1 Visa (doesn't make sense to me....)

- Question on the form: Consulate where Immigrant Visa was issued or USCIS office where status was Adjusted

I sent my I-485 to Chicago Lockbox, so I guess this means the answer is National Benefits Center, correct?

Just for your info, my case is messed up.. ^_^ I submitted my application on April 2006 (yep, two years ago), did my biometrics on the same year.

Sept 06 got a notification my case was transffered to CSC. Jan 07 we moved so I submitted Change of Address (this where the problem starts).

Feb 07, my online case status said they sent a document to me but I never received it. Called them couple times with no results.

April 07 (or was it March?) made an infopass, found out both of my biometrics rejected. So, as suggested, I submitted Police Clearance to CSC.

Sept 07 did 3rd biometrics.

Nov 07 made infopass just to get annoyed by the officer (hahaha... she's really didn't help anything, just told me to wait and try write a letter to CSC). I did write a letter to find out my case status (on the online status is still the Feb 07 status!)

Jan 08 got a repplied letter from CSC, just saying 'check your status online or call customer service' SIGH......

Today I called customer service, they said my case WAS approved in Feb 07 and the card was mailed on Feb 07. I never received both. I freak out! So she said I should submit I-90 (application for replacement card) but since they never got any those mail back so now I gotta submit as if it lost or stollen. And pay $370 (Ugh.... another money). Anyway, as long as I can get the card, I guess I gotta do what I gotta do.

Though I really confused why whenever I had my infopass the lady there said the other thing (they seems couldn't know my case is approved or not), I just want this done...

Sorry for a long story... I hope if someone has similar case, this can help. Do bugs them couple times, I guess it's worth it. :crying:
