Carefully choose between the two. If you're taking a math test and the answer is zero, you don't write N/A. The question about other names used as definitely applicable, even if the answer is "none". The two terms don't carry the same meaning.

I have just had a quick look at the example forms and the word NONE is used - i used these example forms throughout my K1 process and have to say they were very helpful.
