Hi Everyone,

When I had my medical exam in Hong Kong I didn't got any vaccinations. Coz the form from HK Consulate says Vaccination Exempted, meaning that I should have do it here in US. We sent our AOS application on Jan 12, 2006 without vaccination report and we didn't receive an RFE. My questions are how we should know what vaccinations I needed? Should I call the civil surgeon in our area to find out? How much is the cost per shots? Do the civil surgeon will accept medical insurance like Aetna?

here is the info you need, i didnt take shot with doctors listed here i went to public health services is cheaper, i only asked civil surgeon to fill up the form I 693 A vaccination supplement and charged me 25 dlls and didnt accept my insurance ok

The doctor will furnish your with an I-693E in a sealed envelope to submit with your AOS package.

I finally got rid of the never ending money drain. I called the plumber, and got the problem fixed. I wish her the best.

I read the posts about rfes and I693 and civil surgeons but because we didn't get an RFE - I was willing to take a chance (and the wording 'unless already submitted' meant to me that they had what they needed from the consulate.) No one asked about the I693 or vaccination supplement and we got approved. At worse, I rationalized - we will be approved pending the I693 and then and only then we will go and get it! So, if you have a little time to spare and feel lucky ...... You may also do what I did - Nothing - and just take a chance. Good Luck

I read the posts about rfes and I693 and civil surgeons but because we didn't get an RFE - I was willing to take a chance (and the wording 'unless already submitted' meant to me that they had what they needed from the consulate.) No one asked about the I693 or vaccination supplement and we got approved. At worse, I rationalized - we will be approved pending the I693 and then and only then we will go and get it! So, if you have a little time to spare and feel lucky ...... You may also do what I did - Nothing - and just take a chance. Good Luck

I am worried :( , I didn't feel lucky! During my medical exam in Hong Kong I only have Physical Examination, Chest X-ray, VDRL (don't know what it is) and HIV test. That's it! I don't have a copy of my medical report, the doctor didn't gave me one. The doctor only gave me a sealed medical report which I brought during my K-1 interview. I don't have a I-693A or I-693 vaccination supplement that all JV members are talking about. I don't have a medical report to bring if I am going to a civil surgeon here in TX. I am so worried and confused! What documents should I bring when I am going to visit the civil surgeon? If I'll get it done should I send it to Lee Summt without an RFE receipts?

Please help me... I am really confused!

The only thing I brought with me to the civil surgeon was the vacciantion worksheet from the medical abroad. This and the x-ray are the only documents I have from that medical. The rest was put into the brown envelope which I gave to the Embassy.

Since you didn't receive any vaccinations, you don't have any medical documents to bring. If you get the vaccines at your own doctor/clinic, just bring that proof with you to the civil surgeon. It will probably work out cheaper to get the shots at your own doctor and use your insurance, or go a Health Clinic, as the civil surgeon's tend to be money hungry!!

So what I would do in your situation is:

* Make an appointment at your local Doc/Clinic to get the MMR, Tetanus, and varicella vaccine. These are the only ones I've ever gotten throughout our visa journey. Make sure you get some proof of these vaccines.

*Make an appointment to see a civil surgeon to get the I-693a (they will have this form). Call a few and get a good price. Bring your proof of vaccines and passport. I brought my x-ray just incase, but they didn't need that.

*Before the civil surgeon puts the Vaccination Worksheet (I-693A) into the sealed envelope, ask for a copy of this document for your own records.

* If you get an RFE, then mail off the sealed envelope as requested.

*If you do not get and RFE, DO NOT mail this envelope off. Keep it and bring it to your AOS interview.

I had a chickenpox 5 years ago, so I don't need a varicella vaccine. It is correct?

Most Docs will sign off that you've had the chicken pox, and some Docs might want to do a titre test, where they draw a little blood and check to see if you've had the chicken pox. I would imagine a Family Doctor would take your word on the matter though :thumbs:

You should not need a varicella vaccine if you have already had chickenpox. However, some civil surgeons require proof that you've had chickenpox, and most people do not have that. Other civil surgeons make it mandatory to get the varicella vaccine for the I-693A, even if you have had chickenpox. Our civil surgeon didn't need any proof that my husband had had chickenpox when he was younger, she accepted our word, therefore he did not need the varicella vaccine in order to get his I-693A. :)

my fiance had the same problem. the hospital in bangkok sent 2 forms DS-3025 (vaccinations) and DS-3026 (medical exam) to the embassy and told her not to open it. after the k1 interview in bangkok they kept the forms so she didn't think to ask for copies. now she doesn't have any of it for me to apply for AOS. so we had to call the hospital in bangkok who performed her vaccinations and medical exam to obtain the 2 forms. they were very understanding and were willing to help us. they asked if they should either email them to us or mail it. i rather have them mail them to me. after i get them i'm gonna see which vaccinations that she received there and if she has all that's required for the !-693 then i'm just going to send the DS-3025 with the AOS application and hope we get approved. but if not i would have to see a civil surgeon or a public health place here to see if they can help me with the I-693A. hope our experience will give you some reassurance. good luck! i know we need it.
