My fiance does not use her last name on her passport. it is jsut her first name and middle name. Thats what the culture is.. i guess once she is married she will use my last name... so in the question

Surname(as in passport)

what do i write i left it blank and the page throws an error

on line #8, where they ask other surnames, i put down her fathers last name...

I did not have a family name either. What I did was put my first name in the first name column and my second name in the surname column. I wrote a note on a piece of paper explaining that my family do not have a family name, and thats why as they could see every single member of the family had different last name.

My fiance also does not have a last name (only first and middle) for surname we put "none" and haven't had any problems up to date
