2. DS Forms

• 2 DS156 with BARCODE and PICTURE GLUED

• 2 DS 157 - 1 on-line or from the packet and 1 from the bank with a validation code

• 1 DS 156K

3. Original NSO Birth Certificate

4. NBI Clearance - for travel abroad

5. Police Clearances - Laguna and Davao

6. Original copy of Baptismal Cert

7. 3 copies of CENOMAR

8. School Records and Diplomas - Elementary, High School and College

9. Employment Certificate

10. Postal Id & old Employment Id

11. Visa Photos

1. (3) copies of Notarized Affidavit of Support

2. Allan’s pay stubs ( 2005-2007)

3. Medical records ( Newborn Screening result, ultrasound, laboratory test, Medical chart w/ her footprints, our hospital bill/statement of account-paid.

4. Pictures with mommy And Daddy Allan

5. Visa photos

6. Baby book and Immunization book ( Separate folder)

Thanks to Beng and Carl, she let me use her checklist as guidelines. Now here are the documents that am going to bring on my interview. I hope i dont missed anything if yes please let me know.

well, i think you are well-prepared. just some reminders:

1. make sure your folders are half an inch thick and FLEXIBLE.

2. bring a photocopy of the payment slip from Metrobank (P650 fee) for the document verification. then, type or write on that paper your name, passport number, case number, date of interview and name of your petitioner.

3. befor you enter the room of the consul, TAKE A DEEP BREATHE and smile and PRAY!!!

you will make it like what we all did!

12/10/08 - AOS Interview in Los Angeles - APPROVED!!! PRAISE GOD FOREVER!

12/22/08 - Green Card Received - THANKS GOD!!! Thanks our VJ FAMILY!!!

09/14/10 - NOA1 Date (09/15/10 on the uscis website)

09/15/10 - $545 Check Cashed

Hi Beng thanks for reminding me. I will do that, i do have 2 copies of payment slip from metro bank so i might give them one copy and photo copy aswell. My fiance will be with me on my interview.I think before getting inside the consul's room am gonna take a deep breathe and sqeeze my fiancee's hand. :lol::lol::D

thank you for letting us know how to checked our schedule.. i really really do appreciated it!

I have a question here that i hope that somebody could answer me on this. I've worked in japan and i stopped last 2 years ago. When you worked in japan you had only exact 6 months to stay and you have to go home at exact 6 months. I am just wondering if i need still a police clearance sinced i dont stay that long there anyway. I hope that somebody will help me for this...
