Rec'd Packet 3, New form I wasn't aware of "OF-167 Attachment"

Well I thought I had sent all the forms to my fiance' that she needed from me for when packet 3 arrived but there is one inside the packet that I have not heard of or knew nothing about. She said it is form: OF-167 Attachment.

I have looked for this form on the U.S. Dept of States site to see if I can print it off but have had no luck finding it. The closest form I see is an OF-126 and it's not close at all.lol

Anyone have a link where I can obtain this so I can fill it out and send up to her right away instead of having to wait for the mail?

Maybe the form has to come from the US Consulate & it can't be printed from online?

Anyone else received this form in packet 3? Is it something that pertains to military only or a special circumstance form?

My fiance' said it's different than the Affidavit of support that I had sent her and as I understood it this was the only form needed for the "financial support declaration\commitment"?

Feb. 14, 2006 >> Faxed NVC to request change of Embassy for my fiancee as she moved from the Philippines to Canada Jan 20 2006

Feb. 15, 2006 >> NVC had me fax a 2nd request for Req of Change of Embassy.

Feb. 16, 2006 >> Called to ck on 2nd fax request, too late, file sent to US Embassy in Manila.

Feb. 21, 2006 >> Called US Embassy in Manila to ask for Req for change of Embassy to Vancouver. The representative said it shows the request already made. (Evidently from the NVC or Vancouver)

April 3, 2006 >> Received call from Vancouver US Consulate informing me that my fiancee needed to fax them a copy of her Canadian work permit, our case #, current address and phone #. Fiancee faxed requested documents that same day.

April 20, 2006 >> Packet 3 rec'd from Vancouver US Consulate.

April 24, 2006 >> Fiance' faxed DS-230 to Vancouver US Consulate

April 24, 2006 >> I scheduled medical examination for my fiance' for May 1, 2006 $220 Canadian.

OF-167 (Optional Form 167) is one of the forms enclosed in the packet. This form is entitled "Evidence Which May Be Presented to Meet the Public Charge Provisions of Law".

Further, the OF-167's current designation is DS-2000 and it's an informational form - nothing to fill out.

Thanks for the heads up. I knew VJ said nothing about it and I wasn't aware of it. So why do they send the form if it asks essentially the same questions as the i-134? Why did my fiance' receive it and Butuan didnt? Merely curious on the previous two questions. Since it's an optional form I see no reason to send it in.

Hi Patos,

As Yodrak said, OF-167 is just an informational form. If you opened the link I sent you, you will notice that the form does not ask questions, it doesn't ask one to fill up anything and it doesn't get sent. It is just a guide/instruction on how to satisfy the requirement of the law by presenting documents as described in the OF-167.

If Butuan did not receive it, it was probably due to human error. The people working at the embassy forgot to put it inside the packet together with the other forms. It does happen ;)
